Enhancement

1. Support well names greater than 8-chars when reading .UNRST files generated by Intersect.
2. Output well completion status of open or closed to the .UNRST file.

Bug Fixes

1. When reading Intersect/ECL .URST files, always shut a well in if it's surface rate is 0. Do not assign slines to x-flowing closed wells.
2. When reading Intersect/ECL .URST files, and a well completion is repeated, streamlines may have been incorrectly assigned to these wells.
3. Fixed crash when converting .SCH file to .3DSLREC, if user forced monthly timestep size but timesteps in .SCH file were larger.
4. Catch if the rootname in the ECLIPSERST= is the same as the rootname of the 3DSL run. This is not allowed.
