Bug Fixes

1. When SURVEILLANCE=ON and starting from  a .rst file:
   1. Wells on HIST control should use historical rates rather than from the fractional-flow forecast.
   2. Allow wells to switch to BHP or LASTSIMBHP control.
   3. WAFs were not properly updated for wells that switched from LASTVOI to BHP control.
2. For incompressible runs, when checking that each separate ZONENUM has at least one BHP setting, this check was incorrect if all injectors were on PLT= control.
3. For Cartesian grids and wells defined with X,Y,Z= completions, sometimes a completion on the edge of the grid was computed as being outside of the grid.

Enhancements

1. The .waf file now always contains the simulated values at voidage conditions regardless of SURVEILLANCE=ON/OFF.  Thus in surveillance, as long as a well does not switch from voidage control to BHP control, the rates will match historical voidage rates.
2. The .slbun file now always contain the simulated surface and voidage values as well as the historical surface and voidage values.
